A condition has been imposed, for providers of 0107 Assistance with daily personal activities, for risk assessment of participants who choose to be supported by a single worker. A variation condition is imposed for assessment of the remaining elements of Module 4 to be conducted 3 months after service delivery has commenced in class of support 0132. A variation condition is imposed for assessment of the remaining elements of Module 1 to be conducted 3 months after service delivery has commenced in class of support 0104. A variation condition is imposed for assessment of the remaining elements of Module 5 to be conducted 3 months after service delivery has commenced in class of support 0131. A variation condition is imposed for assessment of the remaining elements of Module 2A to be conducted 3 months after service delivery has commenced on 2A Implementing Provider. A variation condition is imposed for assessment of the remaining elements of Module 2 to be conducted 3 months after service delivery has commenced in class of support 0110. A variation condition is imposed for the assessment of Module 5A: Assistance with SIL. The audit must be conducted as part of the provider?s mid-term audit which must commence no later than 18mths after the date of registration.
